Jeff,
Assuming you meant failures, not family, ruptured diaphragm is the killer of these regulators. Old age gets them sooner or later and rots out the diaphragm material. Then it no longer regulates fuel pressure. Additionally fuel passes through the rupture and ends up in the plenum by way of the vacuum line. You can imagine the rest.
